LIFE INSURANCE

Thirty (30) days after you were voted into Union membership, your employer began deducting dues and your $6,000 life insurance coverage went into effect.

Beneficiary Designation
As noted on your signed Application for Membership, your life insurance beneficiary will be disbursed to the same legal beneficiary that is on file with your employer. If you work for Alachua County Library District or Gainesville Housing Authority or If you wish to assign a different designated beneficiary other than the one on file with your employer, you must furnish an original notarized statement to CWA Local 3170. The notarized statement will be placed in your personal file.

When you have been a dues paying member in the Union for five years and you leave or retire from your job, the Union MUST be notified within thirty days in writing for your life insurance policy to remain in effect. At which time, the Union pays the premiums. Failure to notify the Union cancels the life insurance policy.
